Print the 3 forms below (or stop by the store to pick up paper copies).

1. Complete the Photo Release, Application for Assistance, and Authorization to Release Medical Information. 

2. Bring the Photo Release and Application for Assistance to The Cancer Thrift Store of Beaufort.

3. Give the Authorization to Release Medical Information to your doctor. Your doctor will mail medical information detailing your diagnosis and treatment directly to us. 

 

Important: A copy of the Authorization to Release Medical Information along with your official diagnosis must be mailed directly from your doctor to us at:
